Özet

Mesane duvarı kalsifikasyonu tek başına bir tanı değildir; enfeksiyon, kronik inflamasyon, tedavi öyküsü veya kalsifiye tümör gibi farklı süreçleri işaret edebilir. Kontrastsız BT kalsiyumu duyarlı biçimde saptar ve çizgisel, plak benzeri ya da kitle içi dağılımını gösterir. Mesaneden üreterlere uzanan tutulum örüntüsü ve böbrek parankiminin korunması, endemik bölgelerde kronik ürogenital şistozomiyazı düşündürerek genitoüriner tüberkülozdan ayırmaya yardımcı olabilir. Fokal duvar kalınlaşması veya yumuşak doku kitlesi kalsifikasyonla açıklanıp bırakılırsa ürotelyal malignite atlanabilir.

Faz ve pencere

Kontrastsız tanısal
Duvar içindeki kalsiyum, mesane lümeninden daha yüksek atenüasyonlu ince çizgi, plak veya düzensiz odaklar şeklinde seçilir. Kalsifikasyonu göstermek için intravenöz kontrast gerekmez; kalsifiye duvarın yanında kitle kuşkusu varsa yumuşak doku bileşeni ayrı kontrastlı değerlendirme gerektirebilir.

Önerilen pencereler: Batın (G 400 / M 50), Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Çevresel mural kalsifikasyon — mesane duvarını yaygın ve görece simetrik biçimde izleyen ince kabuk veya plaklar.
  • Fokal duvar kalsifikasyonu — sınırlı segmentte kümelenen kalsiyum; komşu duvar kalınlığı ve yumuşak doku bileşeniyle birlikte tanımlanır.
  • Ürotelyal enkrustasyon — kalınlaşmış duvar boyunca çizgisel ve düzensiz yüzeyel kalsifikasyonlar, enfeksiyöz enkrustasyonu düşündürür.
  • Mesane kontraksiyonu — fibrozis veya kronik inflamasyon eşlik ediyorsa düşük hacimli, kalın duvarlı mesane görülebilir.
  • Distal üreterlerde duvar kalsifikasyonları veya darlıkların mesane kalsifikasyonuna eşlik etmesi, mesaneden üreterlere doğru çıkan tutulum örüntüsünde kronik ürogenital şistozomiyazı destekler.
  • Renal eşlik — böbrek ve toplayıcı sistemde kalsifikasyon, skar veya deformasyon alt üriner sistem bulgusunun etiyolojisini değiştirebilir.
  • Kalsifiye kitle — lümene uzanan yumuşak doku ve düzensiz fokal duvar kalınlaşması kalsifiye tümör olasılığını gündeme getirir.

Normalde

Mesane duvarı kalsifikasyonu, uzun süreli enfeksiyonda görülebilen bir görüntüleme bulgusudur. Odağın mesane duvarı veya lümenle ilişkisini çok düzlemli BT değerlendirmesiyle inceleyin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Genitoüriner tüberküloz
mesane değişikliği çoğunlukla böbrek ve üreterde eşlik eden skar, darlık veya kalsifikasyonlarla birlikte görülür.
Enkruste sistit
kalınlaşmış ürotelyuma yapışık düzensiz, yüzeyel kalsifikasyonlar ve üriner enfeksiyon bulguları beklenir.
Ürotelyal karsinom
fokal düzensiz yumuşak doku kitlesi ve duvar invazyonu kalsifikasyondan daha belirleyici bileşendir.
Pelvik radyoterapi öyküsünde mesane duvarı değişiklikleri görülebilir; radyasyon sistitinde kalsifikasyon nadirdir.
Mesane duvarı kalsifikasyonunda şistozomiyaz, genitoüriner tüberküloz, enkruste üropati, radyasyon sistiti ve kalsifiye ürotelyal tümör ayırıcı tanıda yer alır; dağılım ve eşlik eden üriner sistem bulguları nedenleri ayırt etmeye yardımcı olabilir.
