Özet

İzole üretra taşı sıklıkla üst üriner sistemden göç eder; primer taşlar obstrüksiyonun proksimalinde, kronik enfektif darlık veya meatal stenozla ilişkili gelişebilir. Kontrastsız BT, kalsifik odağın lümendeki yerini ve mesane distansiyonu gibi obstrüksiyon bulgularını gösterebilir; taş aramak için kontrast fazı gerekmez. Üretra taşı, üretra lümeni içinde yer alan belirgin kenarlı hiperdensite olarak görünür; prostatik kalsifikasyonlar veya komşu yapılarla karışmaması için lümendeki yeri doğrulanmalıdır.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız pelvis BT'de kalsifik kalkülün mesane çıkımından prostatik ve membranöz segmente, bulber hatta ya da penil üretraya uzanan lümen içi yeri izlenir; kalküller çoğunlukla çevre yumuşak dokudan daha yüksek atenüasyon gösterse de ölçüm bileşim, boyut ve teknik etkenlere bağlıdır; evrensel bir HU tanı eşiği yoktur. IV kontrast üretral taşı saptamak için gerekmez.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Kalkülün üretra merkez hattındaki seviyesi sagittal ve koronal reformasyonlarla prostatik, membranöz, bulber veya penil olarak belirlenir.
  • Darlığın proksimalinde ya da divertikül içinde takılan taşın yanında eşlik eden duvar düzensizliği veya keseleşme bulunabilir.
  • Mesane boynu veya daha distal üretra tıkanırsa mesane distansiyonu, duvar trabekülasyonu ve rezidü taş yükü eşlik edebilir.
  • Yüksek idrar yollarından göç eden izole taşlar tek ve düzgün kenarlıyken, primer taşlar kronik darlık veya divertiküller gibi obstrüksiyon alanlarında birden fazla veya kümelenmiş olarak gelişebilir.
  • Kalkül ile aynı seviyede üretra lümeninde daralma, proksimalde genişleme veya periüretral yumuşak doku değişikliği aranır.
  • Özel multidetektör BT üretrografisi, erkek üretrasının tüm uzunluğunu tek incelemede değerlendirebilir.
  • MDCT üretrografide reforme görüntülerle üretra lümeni ve içindeki kalkülün yeri değerlendirilebilir.

Normalde

MDCT üretrografide normal üretra, mesane boynundan dış üretral meatusa uzanan kontrastla dolu tübüler yapı olarak izlenir. Kalsifik odağın üretra lümeni içinde olup olmadığını doğrulayın; üretra taşı lümen içinde keskin sınırlı hiperdensite olarak tanımlanır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Komşu pelvik kalsifikasyonlar, üretra hattına göre konumları ve morfolojileri ile ayırt edilmelidir.
Üreterovezikal bileşke taşı distal üreterin intramural bölümünde yer alır; üretra ise mesane boynundan dış meatusa uzanan ayrı bir kanaldır.
Kalsifiye üretral divertikül veya yabancı cisim
lümen içinde taş gibi görünebilir; kese duvarı ya da cihazla süreklilik araştırılır.

Tuzaklar

  • Prostatik üretra içindeki kalkül prostat kalsifikasyonu ile aynı seviyede görünebilir; odağın bez dokusuna mı yoksa üretra merkez hattına mı oturduğunu sagittal görüntüde çöz.
  • Vezikal ve üretral kalküllerin ayrımında, taşın vezikal lümende bağımsızlığı veya üretral devamlılığı sagittal ve koronal yeniden yapılandırmalarla doğrulanmalıdır.
  • MDCT ürografi, erkek üretrasının tüm uzunluğunu tek incelemede değerlendirebilir; distal penil üretra da bu değerlendirmeye dahildir.
