Olecranon fracture

Özet

Olekranon, ulnohumeral eklemin arka yüzünü oluşturan ve triseps çekişini taşıyan proksimal ulna parçasıdır; doğrudan darbe veya düşme sonrası kırılabilir. Kompleks veya çok parçalı olekranon kırıklarında BT, kırık morfolojisini ve ara fragmanları daha ayrıntılı değerlendirmeye yardımcı olur; Mayo sınıflaması deplasman, parçalanma ve ulnohumeral eklem stabilitesini temel alır. Koronoid ve radial baş yaralanmalarıyla ulnohumeral uyumun birlikte değerlendirilmesi basit kırık ile kırıklı çıkık örüntüsünü ayırır. Eklem basamağı veya eşlik eden instabiliteyi atlamak, yaralanmanın anatomik kapsamını eksik bırakır.

Okuma sırası

  1. Olekranon konturu. Sagittal rekonstrüksiyonda posterior ulnanın dirsek ucunu bul; trochlear çentiğin arka duvarını oluşturan olekranon konturunu izlemeye başla.
  2. Olekranon kırık hattı. Kortikal kesintinin apeks, gövde veya eklem yüzüne yakın bölümde başladığını belirleyip kırık hattını komşu kesitlerde takip et.

Kontrastsız tanısal
Sagittal ve aksiyel kemik rekonstrüksiyonlarda olekranon korteksini kesen transvers, oblik veya parçalı çizgi; proksimal fragmanın triseps yönünde ayrışması; sigmoid çentik eklem yüzü basamağı ve ulnohumeral hizalanma görülür. Kontrastsız ince kesit BT kemik çizgisi ve fragman geometrisi için tanısaldır; intravenöz kontrast gerekmez.

BT bulguları

  • Olekranon kırıklarında kırık hattının trochlear çentiğe giriş ve çıkış yeri kırık örüntüsüne göre değişebilir.
  • Proksimal fragmanın ana ulnadan ayrışma yönü ve aradaki açıklık, triseps çekişinin oluşturduğu deplasmanla birlikte kaydedilir.
  • Parçalanma, birden fazla kemik fragmanının eklem yüzü ve metafiz arasında nasıl dağıldığı aksiyel-koronal rekonstrüksiyonlarda belirlenir.
  • Sigmoid çentik yüzündeki basamak ya da çökme, trochlea ile eklem uyumunu doğrudan etkileyen eklem içi uzanımı gösterir.
  • Ulnohumeral eklemin merkezlenmesi ve ön-arka yer değiştirmesi, ayrışmış fragmandan bağımsız olarak değerlendirilir.

Ölçütler ve sınıflamalar

Mayo olekranon sınıflaması
Tip I deplase olmayan ve stabil; tip II deplase fakat stabil; tip III deplase ve instabil kırığı tanımlar. A alt grubu parçalanmamış, B alt grubu parçalı örüntüdür; eklem stabilitesi sınıflamanın temel bileşenidir.

Normalde

Normal olekranon, ulna arka-proksimal ucunda sağlam korteksle yükselir ve koronoid ile birlikte trochleanın oturduğu düzgün sigmoid çentiği oluşturur. Aynı seviyede sağlam karşı olekranonun konturunu ve ipsilateral çentik eklem yüzünün sürekliliğini kıyasla; kırıkta kortikal hat, fragman ayrılması veya trochlear yüzeyde basamak görülür.

Ayırıcı tanı

Çocuklarda dirsek travması değerlendirilirken apofizler, sekonder ossifikasyon merkezleri ve normal gelişim varyantları yaşa bağlı kemik gelişimiyle birlikte değerlendirilmelidir.
Koronoid baz kırığı ile olekranon kırığı proksimal ulna üzerindeki farklı bölgeleri tutar; koronoid ve olekranon arasındaki anatomik ayrım değerlendirilmelidir.
Proksimal ulna şaft kırığı
hat olekranon eklem yüzünden uzakta ulna gövdesini izler ve sigmoid çentiği ayırmayabilir.
Triseps insersiyon avulsiyonu
küçük kemik parçası olekranon apeksindeki tendon yapışma noktasındadır; ana eklem yüzü kırığı olmayabilir.
Monteggia kırıklı çıkığında proksimal ulna kırığına radius başı çıkığı eşlik eder.
