Özet

Kemik metastazları ağrıya, patolojik kırığa veya spinal kord basısına yol açabilir. BT kemik yıkımını ve ileri hastalıkta stabilite ile kırık riskini değerlendirebilir; MRG kemik iliği infiltrasyonunu yıkım belirginleşmeden gösterebilir, metastazların yumuşak doku bileşenini BT'den daha iyi tanımlar ve spinal kord ya da sinir basısını değerlendirebilir. İskeletin tümü, incelemeye dahil edilen kemikler üzerinden gözden geçirilebilir. MRG, yüksek yumuşak doku ve kemik iliği kontrastı sayesinde kemik metastazlarının yumuşak doku bileşenini tanımlamada BT'den üstündür; spinal kord veya sinir basısını değerlendirmeyi de sağlar. Vertebra metastazlarında spinal kord veya sinir basısı, patolojik kırık ve spinal instabilite görülebilecek komplikasyonlardır.

Kontrastsız tanısal
Kontrastsız kemik penceresinde kemik yıkımı değerlendirilir; ileri hastalıkta BT ile stabilite ve kırık riski de değerlendirilebilir. Kemik metastazları, belirgin kemik yıkımı ortaya çıkmadan önce kemik iliği infiltrasyonu aşamasında saptanabilir.

BT bulguları

  • Litik medüller odak — tümör hücrelerinin osteoklastları aktive etmesiyle görüntülemede trabeküler yıkım gelişebilir.
  • Geçiş zonunun genişliği ve köşeli sınırlar, lezyonun biyolojik agresifliğini yansıtan önemli BT bulgularıdır.
  • Permeatif osteoliz, kemikte agresif destrüksiyon paternlerinden biridir.
  • Hepatoselüler karsinomun kemik metastazlarında kortikal kemik destrüksiyonu görülebilir.
  • İleri aşamalarda lezyonlar nedeniyle kemik stabilitesinin bozulması ve patolojik kırık riski değerlendirilebilir.
  • Kortikal defektlerden uzayan yumuşak doku kitlesi, özellikle omurga lezyonlarında epidural uzanım ve nöral sıkışma riskini oluşturur.

Tuzaklar

  • Toraks, abdomen ve pelvis BT incelemelerinde omurga ve pelvis kemikleri süperpozisyonsuz değerlendirilebilir; kemik ve yumuşak doku kontrastına göre pencere ayarı yapılabilir.
  • Tedavi yanıtında litik lezyonun dolması veya sklerotik kenar gelişmesi görülebilir; bu nedenle takip BT'si önceki görüntülerle karşılaştırılmalıdır.
  • BT'nin yumuşak doku kontrastı MRG'den düşüktür; MRG kemik metastazlarının yumuşak doku bileşenini tanımlamada BT'den üstündür ve spinal kord veya sinir basısını değerlendirebilir.
  • Spinal kanal veya epidural tümör uzanımı şüphesinde MRG ile spinal kord ya da sinir basısı değerlendirilmelidir; BT'de belirgin bulgu olmaması basıyı tek başına dışlamaz.
