Özet

Ön kolon kırığı, asetabulumun ön kısmında yer alan ve iliak kanatta yukarı uzanan bir segmentin ayrılmasıdır; kırık hattı genellikle iskiopubik ramustan başlar ve iliopektineal çizgide kesintiye yol açar. İnce kesit kontrastsız BT, kırığın ramustan iliuma kesintisiz seyrini, segmental parçalanmayı, quadrilateral yüzün medializasyonunu ve eklem çatı tutulumunu haritalar. İliopektineal çizgideki kesinti ön kolon bileşenini düşündürür; posterior kolondan eşlik eden hemitransvers hat ayrı olarak aranmalıdır. Kolon hattının iliak kanatta kraniyal uzanımı ya da eklem yüzü yer değiştirmesi gözden kaçarsa, kolon kırığı duvar yaralanması olarak yanlış sınıflandırılabilir.

Kontrastsız tanısal
Aksiyel kesitlerde pubik ramus/obturator foramen üst kenarından anterior kolon boyunca ilerleyen kortikal çizgi, anterior asetabular yüz ve iliak kanada doğru takip edilir. Kemik algoritmalı koronal-sagittal rekonstrüksiyonlar kraniyal çıkış düzeyini, iliopektineal çizgi kesintisini, quadrilateral yüzün mediale yer değiştirmesini ve femur başı uyumunu gösterir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

  • Kırık hattı iskiopubik ramustan çıkar ve anterior kolona doğru ilerler; ön kolon kırıklarında ramus kırığı neredeyse her zaman bulunur.

Judet–Letournel temel kırık örüntüleri
Anterior column, pubik ramustan ön asetabuluma geçip ilium içinde yukarı yükselen bir kolon segmentinin ayrıldığı temel örüntüdür. Kırık kraniyal çıkış yüksekliğine ve segmental olup olmamasına göre betimlenebilir; posterior kolon hattı varsa ilişkili örüntü ayrıca belirtilir.

Anterior column/posterior hemitransverse
Ön kolon kırığına, asetabulumun arka kısmından geçen transvers (hemitransvers) bir çizgi eşlik edebilir; bu iki bileşen birlikte ACPHT (ön kolon + posterior hemitransvers) olarak adlandırılan ilişkili bir örüntü oluşturur.

Normalde
