Özet

Transvers asetabulum kırığı, tek bir oblik-yatay hattın asetabulumu ön ve arka kolonlar boyunca geçerek üst iliak-çatı parçasını alt iskiopubik parçadan ayırdığı temel kırık örüntüsüdür. Kontrastsız ince kesit BT, hattın iki kolondan sürekliliğini, yük taşıyan çatıyı kesip kesmediğini ve alt segment ile femur başı arasındaki yer değiştirme ve rotasyonu gösterir; transvers kırıklarda femur başında genel bir medializasyon beklenmez. Hattın eklem çatısıyla ilişkisi ve posterior duvar fragmanı, kırığın sınıflamasını ve eklem uyumunu anlamak için önemlidir. T tipi kırıkların alt kolon ayrılması ve eşlikçi posterior duvar fragmanı, alışılagelen röntgenlerde sınıflandırılmaları zor olduğundan çoklu düzlem rekonstrüksiyonlu BT ile incelenmelidir.

Aksiyel kesitlerde asetabulumu önden arkaya geçen kırık çizgisi anterior ve posterior kolonları aynı örüntüde keser; koronal ve sagittal kemik algoritmalı rekonstrüksiyonlar hattın superior eklem çatısına göre seviyesini, alt segment ile femur başı arasındaki yer değiştirme ve rotasyonu ve eklem uyumunu gösterir. Obturator halka içinde aşağı uzanan ek dikey bacak T tipi uzanımı, posterior rimde ayrı fragman ise ek posterior duvar bileşenini düşündürür. Kırık morfolojisini göstermek için kontrast fazı gerekmez.

BT bulguları

  • Ön-arka kolon geçişi — tek kırık hattı anterior kolondan posterior kolona uzanarak asetabulumun iki kemik sütununu aynı seviyede ayırır.
  • Transvers kırık hattı her iki kolonu da geçerek, superior iliak kanat ve asetabular çatıyı inferior iskium ve pubisle ayırır.
  • Çatı ilişkisi — hattın yük taşıyan superior kubbeyi kesip kesmediği ve subkondral yüzü basamaklandırıp basamaklandırmadığı belirtilir.

Judet–Letournel temel kırık örüntüleri
Transverse, beş temel asetabulum örüntüsünden biridir: tek hat her iki kolonu geçerek eklemi üst iliak/çatı ve alt iskiopubik parçalara ayırır. Posterior rimde bağımsız fragman varsa transverse–posterior wall olarak ilişkili örüntüye geçer; aşağı inen dikey bacak varsa T tipi değerlendirilir.
AO alt tipi: çatıya göre hat seviyesi
AO Surgery Reference hattı eklem çatısına göre transtektal (yük taşıyan çatıyı keser), jukstaktektal (fossa ile çatı komşuluğundan geçer) veya infratektal (fossanın daha altından geçer) olarak tanımlar. Bu morfolojik adlar kırık çizgisinin konumunu belirtir; tek başına tedavi kararı değildir.
Judet–Letournel ilişkili örüntüler
Transvers hatta posterior duvar fragmanı eklenmesi transverse–posterior wall örüntüsüdür; obturator halka içinden aşağı uzanan bacakla kolonların ek ayrılması ise T tipi kırığı düşündürür.

Normalde

Normal asetabulumda anterior ve posterior kolonların kemik konturları kesintisiz olarak aynı eklem yuvasını destekler; iliopektineal ve ilioiskial çizgiler düz ve kopuksuz görünür. Kırık tarafta alt iskiopubik segmentin üst iliak-çatı segmentine göre kaymasını, superior eklem yüzündeki basamaklanmayı ve karşı tarafın merkezlenmiş femur başı ilişkisini kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar
