Özet

Üst ekstremite arter stentleri ve bypass greftlerinde tromboz, stenoz, psödoanevrizma veya enfeksiyona bağlı değişiklikler görülebilir; damar açıklığı ve ekstremite perfüzyonu etkilenebilir. BTA, üst ekstremite damarlarının açıklığını ve anatomisini değerlendirebilir; greftin yapısal bütünlüğü de görüntülemeyle incelenebilir. Önceki incelemeyle karşılaştırma, yeni veya artan perigreft değişikliklerini ve greft açıklığındaki değişimi fark etmeye yardımcı olur. Greft enfeksiyonunda psödoanevrizma gelişebilir ve septik emboliler damar oklüzyonlarına yol açabilir; görüntüleme greftin yapısal bütünlüğünü ve enfeksiyonun yayılımını değerlendirmeye yardımcı olur.

Okuma sırası

  1. proksimal anastomoz ve cihaz başlangıcı. Görüntüleme alanındaki greft veya stentin konumunu ve yapısal bütünlüğünü belirle; BTA'da damar açıklığını ve cerrahi planlama için gerekli anatomik ayrıntıları değerlendir.
  2. greft veya stent lümeni. Greft veya stent lümeninin açıklığını ve düzensizliklerini BTA'da değerlendir; şüpheli kesinti veya daralmayı artefakt olasılığını da gözeterek incele.
  3. distal anastomoz. Greft hattını ve anastomozları değerlendir; BT'de greft bütünlüğü ile anastomotik kaçak veya psödoanevrizma varlığı araştırılabilir.
  4. Greft çevresi. Greft çevresindeki inflamasyon, sıvı ve gazı değerlendir; yeni bulgular veya seri BTA’da bu değişikliklerin artması enfeksiyon şüphesini artırabilir.

Faz ve pencere

Kontrastsız
Kontrastsız BT, cerrahi materyal veya yabancı cisimleri kontrast ekstravazasyonu ya da psödoanevrizmadan ayırt etmeye yardımcı olur; greft lümen açıklığı BTA’da değerlendirilir.
Arteriyel faz tanısal
Arteriyel BTA, üst ekstremite damar açıklığını ve düzensizliklerini; greftte yapısal bütünlük, anastomotik kaçak veya psödoanevrizma varlığını değerlendirmeye yardımcı olur. Greft çevresindeki yumuşak doku değişiklikleri, sıvı veya gaz ile anastomotik kaçak görüntülemeyle değerlendirilebilir.
Gecikmiş
Gecikmiş faz, yavaş ekstravazasyonu göstermeye ve aktif kanama ile psödoanevrizmayı ayırt etmeye yardımcı olabilir.

BT bulguları

  • Stent veya bypass grefti lümeninde ani kontrast kesilmesi oklüzyonu düşündürür; distalde yeniden opasifikasyon kollateral akımla ilişkili olabilir.
  • BTA'da stent lümeninin açıklığı ve düzensizlikleri değerlendirilebilir; artefaktlar damar anormalliklerini taklit edebilir veya maskeleyebilir.
  • Anastomoz çevresindeki kısa, fokal lümen daralması stenozu düşündürür; distal opasifikasyon azalması eşlik edebilir ve önceki inceleme varsa karşılaştırma yapılır.
  • Greft enfeksiyonunda psödoanevrizma görülebilir; görüntüleme anastomotik kaçak veya psödoanevrizma varlığını değerlendirmeye yardımcı olur.
  • Greft çevresinde yeni veya artan sıvı/gaz ve yumuşak doku değişikliği enfeksiyon şüphesini artırabilir; bu bulgular tek başına tanı koydurmaz ve ameliyat sonrası zaman ile seri incelemedeki değişim dikkate alınır.
  • Greft enfeksiyonuna septik emboliler eşlik edebilir ve damar oklüzyonlarına yol açabilir.

Normalde

Greft lümeni boyunca kontrastın devam etmesi açıklığı destekler; anastomozlar, damar duvarı ve çevre yumuşak dokular eşlik eden komplikasyonlar açısından değerlendirilir. Önceki incelemeyle karşılaştırma değişimi gösterir; MIP veya hacim görüntüsündeki kesintiyi oklüzyon saymadan aksiyel kaynak ve çok düzlemli görüntülerde artefaktı kontrol et.

Greft seyri boyunca damar açıklığını ve lümen düzensizliklerini BTA'da değerlendir; anatomik ayrıntılar cerrahi planlamaya katkı sağlar.
Distal embolide greft lümeni açık kalabilirken greft aşağısındaki bir veya birden çok arter dalında oklüzyon görülebilir.
