Özet

Mezenterik ven trombozu, bağırsak venöz çıkışını bozarak mezenterik konjesyon ve bağırsak iskemisine neden olabilen acil bir damar patolojisidir. Portal venöz kontrastlı BT trombüsü doğrudan gösterebilir ve aynı anda bağırsak duvarı ile mezenterdeki etkilerini haritalar. Venöz tıkanmada duvar kalınlaşması ve ödem sık görülür; ileri hasarda duvar kontrastlanması azalabilir, pnömatozis veya portal venöz gaz gelişebilir. Trombüsün küçük dallara uzanımını ya da bağırsak canlılığındaki bozulmayı kaçırmak geri dönüşsüz iskemi tanısını geciktirebilir.

Faz ve pencere

Portal tanısal
Portal venöz fazda vena mesenterica superior veya dallarında kontrastlanan kanın içinde parsiyel/tam düşük atenüasyonlu dolum defekti aranır; venöz duvar çevresinde kontrastlanma ve damar çapında genişleme eşlik edebilir. Aynı fazda mezenterik ödem, bağırsak duvarı kalınlaşması ve kontrastlanma paterni değerlendirilir.
BTA tanısal
Akut mezenter iskemi kuşkusu varsa arteriyel BT anjiyografi SMA açıklığını, arteriyel emboli/trombozu ve bağırsak duvarının erken perfüzyonunu göstererek venöz tıkanma ile eşlik eden arteriyel hastalığın ayrımına katkı sağlar; bu seri venöz trombüsün portal fazdaki değerlendirmesinin yerine geçmez.

Önerilen pencereler: Batın (G 400 / M 50), Anjiyo (G 600 / M 150).

BT bulguları

  • SMV dolum defekti — vena mesenterica superior lümeninde kontrastlanan kanla çevrili trombüs; ana gövde veya dallar etkilenebilir.
  • Venöz tıkanma yayılımı — trombüsün ileokolik, jejunal/ileal dallara, splenik vene ya da portal vene uzanması.
  • Mezenterik konjesyon — tromboze venlerin çevresinde ödemli, bulanık mezenter yağ dokusu ve belirginleşmiş venöz arkadlar.
  • Bağırsak duvarı ödemi — etkilenen segmentte simetrik kalınlaşma, submukozal ödem ve katmanlı hedef görünümü.
  • Duvar hipoperfüzyonu — bağırsak duvarının komşu canlı segmentlere göre belirgin az kontrastlanması ileri iskemi kuşkusunu artırır.
  • Mezenterik serbest sıvı — venöz basınç artışıyla etkilenen anslara komşu sıvı veya yeni asit gelişmesi.
  • İleri iskemi işaretleri — bağırsak duvarında pnömatozis, mezenterik venlerde/portal dallarda gaz veya duvar bütünlüğü kaybı.

Normalde

Normal portal venöz BT'de SMV, ince bağırsak mezenteri içinde kontrastla dolu, çevre venöz dalları birleşerek portal vene ulaşan kesintisiz bir gövdedir; ince bağırsak duvarı düzgün kalınlıkta ve homojen kontrastlanır. Aynı mezenterik segmentte ven lümeninin açıklığını, eşlik eden arterlerin kontrastını ve duvar kontrastlanmasını komşu sağlam ansla kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Arteriyel mezenter iskemi
SMA içinde emboli/trombüs veya yaygın arteriyel daralma görülür; bağırsak duvarı hipoperfüzyonu venöz tıkanma bulguları olmadan da gelişebilir.
İnflamatuvar/enfeksiyöz enterit
segmental duvar kalınlaşması ve hiperemi yapabilir; mezenterik ven içinde gerçek dolum defekti beklenmez.
Strangüle ince bağırsak obstrüksiyonu iskemiye yol açabilir; obstrüksiyonda artan lümen basıncı önce venöz çıkışı, ardından arteriyel akımı engeller.
Portal venöz gazın başka nedeni
pnömatozisle birlikte görülebilir, ancak tek başına mezenterik ven trombozu tanısı değildir; mezenterik damar lümenleri ayrıca incelenmelidir.

Tuzaklar

  • Kontrast akım dinamiği nedeniyle dolum defekti kuşkusu doğabilir; trombüsün ardışık kesitlerde devam ettiğini ve dallarda da bulunup bulunmadığını doğrulayın.
  • Bağırsak duvarı kalınlaşması venöz iskemiye özgü değildir; trombüs, mezenterik konjesyon ve duvar kontrastlanmasını birlikte arayın.
  • Pnömatozis veya portal venöz gazı tek başına transmural nekroz kabul etmeyin; duvar kontrastlanması, serbest hava, ans dilatasyonu ve klinik bağlamla birlikte yorumlayın.
