Özet

Posterior fossa araknoid kisti, araknoid zar yaprakları arasında BOS içeren, çoğu kez tesadüfen saptanan benign ekstraaksiyel bir oluşumdur. Beyin MR görüntülemesi kistin BOS sinyallerine uyan yapısını ve komşu serebellar yapılar ile dördüncü ventrikülde oluşturduğu yer değiştirmeyi gösterir. Kist büyüdüğünde dördüncü ventrikül çıkışını daraltıp obstrüktif hidrosefaliye yol açabilir. Yalnızca retro serebellar BOS alanını görmek yeterli değildir: bası ve ventrikül etkisini gözden kaçırmak kisti basısız sisterna magna genişlemesiyle karıştırabilir.

Faz ve pencere

Kontrastsız tanısal
Beyin penceresinde düzgün sınırlı ekstraaksiyel koleksiyon BOS'a yakın hipodens görünür; kistin konumuna göre serebellar hemisfer, vermis, beyin sapı veya dördüncü ventrikül itilmiş ya da basılanmış olabilir. Basit araknoid kistler BOS sinyallerine uyan homojen bir yapı sergiler ve karakterizasyonu için kontrast madde genellikle gerekli değildir.
Kemik algoritması tanısal
Komşu oksipital kemikte uzun süreli basıya eşlik eden düzgün iç tabula çökmesi veya kontur uyumu aranır; bu görünüm kistin yerini ve kronik basısını destekler, tek başına kist tanısı koydurmaz.

Önerilen pencereler: Beyin (G 80 / M 40), Kemik (G 1800 / M 400), Subdural (G 200 / M 75).

BT bulguları

  • Ekstraaksiyel morfoloji — koleksiyon serebellar yüzeyi dıştan sarar veya sisternal aralığı doldurur; parankim içinde kavite oluşturmaz.

Normalde

Normal posterior fossada serebellar yapılar ve dördüncü ventrikül simetrik ve dışbükey konturlarını korur; ana morfolojik özelliklerin tanımı anomali ayrımında kritiktir. Kist tarafındaki serebellar yüzeyin düzleşmesini, dördüncü ventrikülün eksenini ve komşu yapıların yer değiştirmesini değerlendir; MR'da kistin BOS ile sinyal eşleşmesine dikkat et.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
