Özet

BOS şantı olan hastada yeni subdural koleksiyon, aşırı drenajla ilişkili higroma, kanama veya bu süreçlerin birlikte bulunması açısından dikkatle incelenmelidir. Akut kafa travması bağlamında BT, subdural hematom gibi acil nöroşirürjik tedavi gerektirebilecek intrakraniyal lezyonların saptanmasında temel görüntüleme yöntemidir. Ventrikül kalibresi ve şant ucu, koleksiyonun olası mekanizmasını anlamaya yardım eder; tek başına neden-sonuç ilişkisi kurdurmaz. Büyüyen hematom, orta hat kayması veya sisternal bası hızla kötüleşebilecek intrakraniyal basınç artışını işaret eder.

Kontrastsız tanısal
Subdural koleksiyon kalvaryum iç yüzünü izleyen hilal biçimli ekstraaksiyel alan şeklindedir; BOS benzeri sıvı düşük atenüasyonlu, akut kan hiperdens, kronik kan daha düşük veya heterojen yoğunlukta olabilir. Kanamanın evresini yalnız HU ile kesin tarihlendirmeyin; rutin kontrastlı faz gerekmez.

BT bulguları

  • Konveksite boyunca hilal biçimli koleksiyon; sütürleri aşabilir, dural yansımalar boyunca yayılımı değerlendirilir.
  • Higroma genellikle BOS ile benzer atenüasyonda ve beyin parankimine göre hipodens görünür; hematom akut dönemde hiperdens, kronikleşirken izodens veya hipodens olabilir.
  • Karışık yoğunluk, membran veya septa kronikleşmiş/tekrarlayan kanamayı ya da farklı yaşlardaki bileşenleri düşündürür; kesin yaş tayini sağlamaz.
  • Bilateral koleksiyonlar orta hattı belirgin kaydırmadan konveksite sulkuslarını basabilir.
  • Tek taraflı ya da kalın koleksiyonda komşu ventrikül basısı, sulkus silinmesi ve orta hat kayması ölçülüp tarif edilir.
  • Aynı incelemede ventrikül boyutu, kateter ucunun yeri ve bazal sistern açıklığı koleksiyonun klinik etkisini tamamlar.

Normalde

Fizyolojik durumda gerçek bir subdural boşluk bulunmadığı bildirilmiştir; normal incelemede hilal biçimli ekstraaksiyel koleksiyon ve kortikal bası beklenmez. Subdural koleksiyonun etkisi, orta hat yer değiştirmesi ve ventrikül basısı gibi eşlik eden bulgularla değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Bilateral subdural koleksiyonlarda orta hat yer değiştirmesi görülebilir; kitle etkisini orta hat ve ventrikül basısı gibi eşlik eden bulgularla değerlendirin.
  • Kronik subdural hematom beyin parankimine izodens görünebilir; kortikal yüzeyin içe itilmesi ve membran/septa ipucu verir.
  • Higroma ile kronik hematomun atenüasyonu örtüşebilir; tek bir yoğunluk değerinden sıvının niteliği veya yaşı çıkarılamaz.
  • Şant varlığı koleksiyonun otomatik olarak aşırı drenaja bağlı olduğunu göstermez; travma ve eşlik eden kanama nedenlerini de arayın.
  • Özellikle bebeklerde, subdural higromayı diğer patolojilerden ayırmak için başlangıç BT'sinden sonra baş MR'ı gerekebilir.
