Özet

Hemorajik dönüşüm, iskemik olarak hasarlanmış beyin dokusu içinde peteşiyal kan ürünü veya parankimal hematom gelişmesidir; spontan evrimle ya da reperfüzyon sonrasında görülebilir. Kontrastsız BT, infarkt alanındaki yeni hiperdens kanı ürünü, parankimal hematomun kitle etkisi ve ventriküler uzantılarını saptamak için en sık kullanılan birincil görüntüleme yöntemidir. Kanın infarkt dışına taşması veya kitle etkisi oluşturması kötüleşme açısından kritik bulgudur. Anjiyografi sonrası iyot birikimi kanı taklit edebileceğinden, belirsiz hiperdansiteyi yanlış sınıflamak klinik kararları etkileyebilir.

Faz ve pencere

Kontrastsız tanısal
İnfarktla uyumlu hipoattenüasyon veya gri-beyaz sınır kaybı içinde yeni hiperdens peteşiler ya da daha homojen, sınırlı kan pıhtısı aranır. Kontrastsız BT ile ECASS II sınıflamasına göre infarkta oranla hematom büyüklüğü (%30 eşik) ve kitle etkisi (orta hat kayması, ventriküler bası) değerlendirilir; akut kanama tespiti için ek kontrast uygulanmaz.
Çift enerjili BT
Yakın zamanda kateter anjiyografisi veya intra-arteriyel iyot verilmişse, çift enerjili BT'nin iyot haritası ve sanal kontrastsız rekonstrüksiyonları kontrast boyanmasını kan ürününden ayırmaya yardım edebilir; standart tek enerjili geç kontrast fazı bunun yerine geçmez.

Önerilen pencereler: Beyin (G 80 / M 40), Subdural (G 200 / M 75), İnme (G 40 / M 40).

BT bulguları

  • İskemik alan içine dağılmış noktasal veya çizgisel hiperdens peteşiler.
  • İnfarkt sınırları içinde birleşen peteşiler; belirgin yer kaplayıcı etki olmaması.
  • İnfarkt alanında daha homojen, sınırlı hiperdens parankimal pıhtı ve çevresinde hafif bası.
  • Geniş infarkt alanının önemli bölümünü dolduran hematom ve belirgin kitle etkisi.
  • İntraventriküler kanama varsa lateral ventrikül, üçüncü ventrikül veya dördüncü ventrikülde hiperdens kan.
  • Ventrikül komşuluğundaki kanın koroid pleksus kalsifikasyonu veya kontrast boyanmasından ayrılması gereği.
  • Hematoma komşu sulkus silinmesi ve septum pellucidum düzeyinde orta hat yer değiştirmesi.

ECASS II hemorajik dönüşüm sınıflaması
Yaygın kullanılan ECASS II radyolojik alt gruplarında HI1 kenar peteşileri, HI2 infarkt içi birleşen peteşiler ve kitle etkisi olmamasıyla ayrılır. PH1 infarktın %30 veya daha azını kaplayan pıhtı ve hafif kitle etkisi; PH2 %30'dan fazlasını kaplayan pıhtı ve belirgin kitle etkisidir.

Normalde

Normal parankimde infarkt içinde gelişmiş yeni hiperdens odak veya kan ürününe bağlı lokal doku basısı bulunmaz; ventrikül içi BOS dansitesi homojendir. Şüpheli hiperdensitelerin kanama olup olmadığını ayırt etmek için kontralateral homolog bölgelerle yoğunluk kıyaslaması yapılır; kitle etkisi yönünden septum pellucidum sapması, ventriküler deformite ve bazal sistern basısı incelenir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Primer intraparankimal hematom
başlangıçta damar alanına uyan iskemik hipoattenüasyonla çevrelenmeyebilir; derin hipertansif tipik yerleşim veya lobar hematom özellikleri aranır.
İyot kontrastı boyanması
anjiyografi sonrası kortikal/giriform hiperdansite yapabilir; çift enerjili BT'de iyot haritasında görünür ve sanal kontrastsız görüntüde kaybolabilir.
Kalsifikasyon
çoğu kez önceden mevcut, çok yoğun ve anatomik olarak tipik bir odaktır; önceki görüntü ve çift enerjili kalsiyum ayrıştırması yardımcıdır.
Hemorajik venöz infarkt
kanama venöz drenaj alanında, sıklıkla ödemle birlikte ve arteriyel infarkt topografisine uymayan biçimde yerleşir; venöz sinüs trombozu araştırılır.

Tuzaklar

  • Tek enerjili BT'de anjiyografi sonrası iyot ve kan hiperdens görünebilir; işlem zamanını öğrenin ve ayırım klinik açıdan önemliyse DECT veya takip görüntülemesini kullanın.
  • HI peteşileri ile PH hematomunu yalnız parlaklığa göre ayırmayın; birleşme, pıhtı biçimi ve kitle etkisi sınıflamanın temelidir.
  • İnfarkt alanını belirlemeden tüm hiperdensiteyi dönüşüm olarak adlandırmayın; damar kalsifikasyonu, eski kanama ve primer hematomla anatomik devamlılığı kontrol edin.
