Özet

Ortak kavite, gelişim sırasında koklea ile vestibülün ayrı bölümler halinde oluşmadığı konjenital iç kulak malformasyonudur. Kontrastsız ince kesit temporal kemik BT, birleşik boşluğu ve iç mimarinin yokluğunu gösterir; iç akustik kanalın boşluğa nereden açıldığı ayırıcı tanıda yararlıdır. Tanıyı koklear aplazideki geniş vestibülden ayırmak, mevcut anatominin doğru tanımlanması için önemlidir. Ortak kavite, koklear aplazi ve geniş vestibülden ayrılmalıdır; doğru sınıflandırma işitme rehabilitasyonu seçeneklerinin belirlenmesinde belirleyicidir.

Kontrastsız tanısal
Kemik algoritmalı ince kesit BT, koklea ve vestibülün tek bir boşluk oluşturup oluşturmadığını ve iç mimarinin durumunu değerlendirmek için temel görüntüleme yöntemidir.

Önerilen pencereler: Kemik (G 1800 / M 400).

BT bulguları

  • Koklear kanal ve vestibül iki ayrı kemik labirent yapısı olarak seçilemez; aralarında bölme bulunmayan tek boşluk vardır.
  • Kavite içinde normal koklear spiral dönüşler, modiolus ve interskalar septalar izlenmez.
  • İç akustik kanal fundusu çoğunlukla kavitenin merkezine açılır; bu yerleşim birleşik yapıyı destekler.
  • Semisirküler kanallar tam gelişmiş, hipoplastik veya kısmen gelişmemiş olabilir.
  • İç kulak malformasyonları tek taraflı veya çift taraflı görülebilir; genetik kökenli olgularda simetrik tutulum beklenir.
  • Koklear aplazi ve dilate vestibülden ayırımda kalan labirent anatomisi ile kanal fundusunun yönü birlikte incelenir.

Sennaroğlu kokleovestibüler malformasyon sınıflaması
Ortak kavite, koklea ile vestibülün ayrışmadığı ve iç mimarisi olmayan birleşik boşluk şeklindeki ayrı malformasyon kategorisidir; çevre semisirküler kanallar farklı derecelerde gelişmiş olabilir.

Normalde

Normal aynı düzeyde koklear spiral boşluk, vestibülden kemik sınırlarla ayrılır; vestibülün posteriorunda semisirküler kanallar ayrı halkalar halinde izlenir. Ortak kavitede bu koklea-vestibül sınırı ve koklear dönüşler yoktur; iç akustik kanalın fundusunun boşluğun merkezine açılması ve karşı tarafın düzenli bölümlenmesiyle karşılaştırılır.

Ayırıcı tanı

Koklear aplazi ve dilate vestibül
koklea yoktur ama geniş vestibül tek başına kalır; iç akustik kanal fundusu merkeze değil vestibül tarafına yönelir.
İnkomplet partisyon tip I
kistik koklea vestibülden ayrı kalır; yalnız modiolus ve iç bölmeler kayıptır.
