Özet
Peritoneal karsinomatozis, çoğunlukla gastrointestinal veya over kaynaklı tümör hücrelerinin periton, omentum ve mezenter yüzeylerine yayılmasıdır. Kontrastlı BT, implantları bölge bölge haritalar; primer odağı, asiti, barsak serozası ve mezenter tutulumunu göstererek hastalık yükünün ve olası obstrüksiyonun değerlendirilmesini sağlar. Özellikle küçük barsak serozası ve mezenter kökündeki küçük implantlar BT’de gözden kaçabilir. Yayılımın eksik haritalanması görüntüleme ile tahmin edilen hastalık yükünü ve cerrahi planlamayı etkileyebilir; serozal implantlar barsak lümeninde darlık ve obstrüksiyona yol açabilir.

BT bulguları
- Peritoneal implant — solid, sıklıkla heterojen kontrastlanan nodül, plak veya konfluent kitle parietal periton ya da ligament boyunca yerleşir.
- Peritoneal yüzey kalınlaşabilir ve kontrastlanabilir; görünüm tek başına primer süreç veya tanı belirlemez.
- Omental infiltrasyon — erken hastalıkta yağ dansitesi artıp retikülonodüler görünür; ileri yayılımda yumuşak doku plağı omental cake oluşturur.
- Mezenterik implantların kesitsel görüntülemeyle değerlendirilmesinde doğruluk sınırlı olabilir; özellikle küçük barsak ve mezenter hastalığı gözden kaçabilir.
- İnce barsak serozal implantları BT’de gözden kaçabilir; bir çalışmada PET/MR bir olgudaki lezyonu saptarken BT saptamamıştır.
- Peritoneal karsinomatozise malign asit eşlik edebilir.
- Akış ve rezorpsiyon reseslerinde ekim — Douglas/retrovezikal boşluk, ileoçekal bölge, sigmoid mezokolon, sağ parakolik oluk, Morrison cebi, büyük omentum ve sağ subdiyafragmatik yüzey sistematik incelenir.
- Visseral yüz scalloping’i — karaciğer veya dalak konturundaki dıştan çentiklenme, kitle etkili müsinöz asit ve yüzey implantlarıyla görülebilir; psödomiksoma peritonei önemli bir ayırıcı tanıdır.
- PCI, hastalığı 13 abdominal bölgede her bölge için tümör boyutuna göre 0–3 puanlayarak değerlendirir.

Normalde
Normal BT’de periton genellikle seçilemeyecek kadar ince, omentum yağ dansitesinde ve mezenterik yapraklar birbirinden ayrışmış görünür; peritoneal implant veya patolojik miktarda asit bulunmaz. Kıyaslamada implantları tek bir genel yaygınlık ifadesine indirgemeden subfrenik alanlar, parakolik oluklar, omentum, mezenter, serozal yüzeyler ve pelvis boyunca tek tek arayın.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı
- Tüberküloz peritoniti
- düzgün belirgin peritoneal kontrastlanma, santral nekrozlu lenf nodları ve omental bulanıklık bulunabilir; karsinomatoziste nodüler-irregüler implantlar daha olağandır.
- Peritoneal mezotelyoma
- epiteloid tipte asit, yaygın plak benzeri peritoneal kalınlaşma, mezenter infiltrasyonu, anslarda fiksasyon ve omental cake görülebilir; görünüm karsinomatozisle örtüşür, asbest öyküsü veya diğer klinik bulgular yalnızca ipucu sağlar.
- Psödomiksoma peritonei
- düşük dansiteli kitle etkili müsin, septalar ve karaciğer/dalak yüzey scalloping'i ile seröz implantlardan ayrılır.
- Peritoneal lenfomatozis
- periton ve omentumda diffüz kalınlaşma, mezenterik lenfadenopati ve jejunum duvarında kalınlaşma görülebilir.
- Peritoneal lenfatik malformasyon (lenfanjiyom)
- ince duvarlı ve septalı, sıklıkla multiloküle düşük dansiteli kistik oluşumlar görülür; solid peritoneal implant görünümü tipik değildir.
Tuzaklar
- Küçük implantlar özellikle ince barsak serozası ve mezenter kıvrımlarında BT’de seçilemeyebilir; negatif BT küçük veya ince yüzey implantlarını dışlamaz.
- Erken veya düşük hacimli peritoneal hastalık BT’de gözden kaçabilir; bu nedenle omental tutulum değerlendirmesi yalnız belirgin konfluent cake ile sınırlanmamalıdır.
- Pelvikte loküle asit kistik kitle sanılabilir; duvarında implant olup olmadığını ve komşu peritoneal yüzeylerle devamlılığını inceleyin.
- Scalloping karsinomatozise özgü değildir; belirgin müsin birikimiyle pseudomyxoma düşünülmeli ve primer apendiks/adneks odağı araştırılmalıdır.
