Özet

Lateral kondil kırığı çocuk dirseğinde sık görülen, fragmanı kapitellum ve trochleanın eklem kıkırdağına kadar uzanabilen bir yaralanmadır. İnce kesit kontrastsız BT, kemikleşmiş fragmanın yer değiştirmesini, rotasyonunu ve kırık hattının kemik eklem yüzüyle ilişkisini belirginleştirir; kıkırdak menteşeyi doğrudan göstermeyebilir. Ayrışma yalnız metafizer aralık ölçüsüyle değil, fragmanın dönmesi ve eklem yüzünün bütünlüğüyle birlikte değerlendirilir. Lateral kondil kırıklarında kaynamama, yanlış kaynama, kubitus valgus ve “fishtail” deformitesi bildirilen komplikasyonlardandır.

Kontrastsız tanısal
Çocuk lateral kondil kırıklarında stabiliteyi değerlendirmek için BT, ultrasonografi, MRG ve iç oblik radyografi kullanılabilir; radyografiler kırık hattının eklem kıkırdağına uzanımını değerlendirmede yetersiz kalabilir.
Kemik algoritması tanısal
Çocuk lateral kondil kırıklarında radyografi, eklem kıkırdağının bütünlüğünü değerlendirmede sınırlı kalabilir; MRG, troklear kıkırdak zincirinin bütünlüğünü değerlendirmede radyografiden daha duyarlıdır.

  • Lateral kondil-metafiz oblik hattı — kırık lateral kolon boyunca proksimale yükselir; çizginin trochlear olukla ilişkisini izleyin.
  • Kapitellar eklem yüzü uzanımı — fragman radius başıyla eklem yapan yüzeyi taşıyabilir ve eklem içi basamak oluşturabilir.
  • Trohlear oluk ilişkisi — kırık çizgisinin oluğun lateralinde mi yoksa oluğa doğru mu ilerlediği Milch anatomik ayrımını belirler.
  • Fragman ötelemesi ve rotasyonu — lateral veya posterolateral yer değiştirme yanında fragmanın eklem yüzünü öne/arkaya çevirmesi ayrıca kaydedilir.

Jakob
Jakob sınıflamasında tip II ve tip III lateral kondil kırıkları ayrı tipler olarak yer alır. Lateral kondil kırıklarında deplasman miktarı görüntülemeyle ölçülebilir; radyografiler deplasmanı olduğundan az gösterebilir.
Milch
Tip I’de kırık hattı trochlear oluğun lateralinden ilerler; tip II’de oluğa uzanır veya oluğu keser. Bu sınıflama kırık hattının anatomik seyrini anlatır, deplasman ölçüsü vermez.
Song (radyografik örüntü ve stabilite sınıflaması)
Song evre II–III kırıklarında sınıflama kıkırdak menteşe bütünlüğünü kesin olarak göstermez; bir çalışmada bu evrelerdeki 62 kırığın 2’sinde (%3) menteşenin tam kopuk olduğu görülmüştür. Bir kohortta evre II–III kırıkların yalnız %3'ünde kıkırdak menteşenin tam kopukluğu bildirilmiştir.

Normalde

Çocuk dirseğinde gelişimsel apofizlerin olağan yerleşim ve yapılarını bilmek, olası kırık, avulsiyon veya apofiz ayrışmalarını tanımaya yardımcı olur. Aynı tarafın ardışık koronal-sagittal görüntülerinde eklem yüzü çizgisinin düzgünlüğünü, karşı dirsekte ise kondil konturu ve trochlear oluğun şeklini karşılaştırın.

Ayırıcı tanı

Suprakondiler humerus kırığı
hat kondillerin üstündeki metafizde kalır; lateral kondiler fragman ve eklem yüzü ayrışması yoktur.
Lateral epikondil avulsiyonu değerlendirilirken lateral epikondilin çok parçalı gelişimsel bir varyant olabileceği de göz önünde bulundurulmalıdır.
Distal humerus fizis ayrışması
tüm epifiz metafizden ayrılabilir; tek lateral kondiler parça ile sınırlı olmayan deplasman ve bazen metafizer köşe fragmanı görülür.
Kapitellum osteokondral kırığı
fragman çoğunlukla kapitellar eklem yüzüne sınırlıdır; lateral metafizden yükselen tipik kondiler hat yoktur.
Lateral epikondilin çok parçalı görünümü gelişimsel bir varyant olabilir.

Tuzaklar

  • Kıkırdak bölümü kemikleşmemiş çocukta küçük kemik aralığı eklem yüzünün sağlam olduğunu kanıtlamaz; BT'nin kıkırdak menteşeyi göstermediğini rapor sınırında belirtin.
  • Radyografiler lateral kondil kırıklarında yer değiştirme miktarını olduğundan az gösterebilir; MRG ölçümleri gerçek yer değiştirme değerlerine daha yakındır.
  • Milch tipini fragmanın ne kadar yer değiştirdiğiyle eşitlemeyin; bu sınıflama hattın trochlear oluğa göre seyrini anlatır.
  • Lateral epikondilin çok parçalı görünümü gelişimsel bir varyant olabileceğinden, bunu travmatik kırıkla karıştırmayın.
