Özet

Pediatrik akut osteomiyelitte azalmış uzuv fonksiyonu, ateş ve lokalize ağrı sıklıkla görülür ve enfeksiyon ciddi morbiditeye yol açabilir. Kontrastsız kemik algoritmalı BT kortikal erozyonu, kronik olguda sekestri, involukrumu ve kloakayı iyi gösterir; yumuşak doku koleksiyonunun sınırları için kontrastlı inceleme ve erken kemik iliği tutulumu için MR daha duyarlıdır. Uzun kemikte medüller enfeksiyon korteksi aşarak subperiostal ve kas içi yayılım gösterebilir. BT ve MR, osteomiyelit yayılımını ve yumuşak doku komplikasyonlarını değerlendirmeye yardımcı olur ancak görüntüleme bulguları klinik ve laboratuvar verileriyle birlikte yorumlanmalıdır.

Faz ve pencere

Kontrastsız tanısal
Kemik penceresinde medüller litik destrüksiyon, kortikal erozyon/kesinti, kortikal kalınlaşma ve yoğun sekestr aranır; yumuşak doku penceresinde yağ planı kirlenmesi, gaz ve sıvı yoğunluklu koleksiyon görülebilir. BT, kemik enfeksiyonunu değerlendirmede diğer modalitelere göre daha düşük duyarlılığa sahiptir; bu nedenle BT'nin normal olması osteomiyeliiti kesin olarak dışlamaz.
Kemik algoritması tanısal
İnce kesit multiplanar görüntüler sekestri çevreleyen reaktif yeni kemik kabuğunu (involukrum), kortikal drenaj açıklığını (kloaka) ve korteks boyunca destrüksiyonun uzunluğunu gösterir.
Kontrastlı tanısal
BT, erken kemik erozyonunu tespit etmek ve sekestr, yabancı cisim veya gaz oluşumunu belgelemek için kullanışlı bir yöntemdir. Erken kemik iliği tutulumu için MR daha duyarlıdır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Medüller osteoliz — distal femur veya proksimal tibia medullasında düzensiz, coğrafi olmayan kemik kaybı.
  • Kortikal erozyon/destrüksiyon — medüller odağa komşu kortekste incelme, oyulma veya tam kat kesinti.
  • Periostal yeni kemik — kemik yüzeyine paralel reaktif tabaka; kronik enfeksiyonda kalınlaşmış kabuk biçimini alabilir.
  • Sekestr — çevresinde lusent granülasyon/rezorpsiyon halkası bulunan, çevre kemiğe göre yoğun nekrotik kemik fragmanı.
  • Gaz oluşumu — BT ile saptanan, enfekte kemik iliği veya yumuşak dokularda görülen hava yoğunluklu alanlar.
  • Sekestrasyonun saptanmasında BT kullanışlıdır; ancak osteomiyelit tespitinde MR en duyarlı ve özgül yöntemdir.
  • Subperiostal koleksiyon — korteksi dıştan takip eden, periostu kaldıran sıvı yoğunluklu koleksiyon.
  • Komşu yumuşak doku yayılımı — kas planlarında ödem, yağ dokuda çizgilenme, gaz veya organize koleksiyon.

Normalde

Erken kemik erozyonu — BT ile saptanabilen, kortikal bütünlüğün bozulduğu ve medüller yıkımın başladığı erken dönem radyolojik bulgulardan biridir. Osteomiyelit tanısında ve hastanın yönetimi için çeşitli görüntüleme tekniklerinin kombinasyonu gerekebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Brodie apsesi
metafizde iyi sınırlı litik kavite ve belirgin sklerotik duvarla sınırlı subakut enfeksiyon biçimidir.
Subakut kırık/iyileşen travma

Tuzaklar

  • BT, kemik enfeksiyonunu değerlendirmede diğer modalitelere göre daha düşük duyarlılığa sahiptir; bu nedenle BT'nin normal olması osteomiyeliiti kesin olarak dışlamaz.
