Özet

Anterior glenohumeral çıkık, humerus başının glenoidin önüne geçtiği travmatik eklem yaralanmasıdır ve akut omuz travmasında sık görülür. Kontrastsız BT, özellikle redüksiyon öncesi/sonrası eklem uyumunu, glenoid kenar kırığını ve humerus başı impaksiyonunu kemik ayrıntısıyla gösterir. Bu kemik yaralanmalarının boyutu tekrarlayan instabilite riskinin değerlendirilmesinde önem taşır. Eşlik eden kırık veya eklem içi parça atlanırsa çıkığın yalnızca hizalanma sorunu gibi raporlanması yaralanma kapsamını eksik bırakabilir.

Okuma sırası

  1. Glenoid-humerus başı hizası. Aksiyel kesitte glenoid fossayı bulun ve humerus başı merkezinin bu çanağın önüne düşüp düşmediğini takip edin; eklem yüzlerinin örtüşmesini aynı kesitte gösterin.
  2. Anteroinferior glenoid. Anteroinferior glenoid kenarını kesintisiz korteks boyunca izleyin; kopmuş fragmanın yatağını ve eklem yüzü kaybını karşı tarafla kıyaslayın.

BT bulguları

  • Anterior yer değiştirme — humerus başı glenoid fossanın önünde, sıklıkla anteroinferior konumdadır.
  • Eklem uyumsuzluğu — glenoid eklem yüzü ile humerus başı merkezi karşı karşıya gelmez; redüksiyon sonrası tekrar uyum kontrol edilir.
  • Kemik Bankart, anteroinferior glenoid kenarının kırığıdır ve travmatik anterior omuz çıkığıyla sık ilişkilidir. Glenoid kenarındaki kemik kaybı, tekrarlayan instabilite için önemli risk faktörlerinden biridir.
  • Hill-Sachs impaksiyonu — humerus başının posterolateral eklem yüzünde çökme oluğu.
  • Eklem içi serbest parça — glenoid veya humerus başı kırığından kopmuş kemik/osteokondral fragmanın eklem aralığında yer değiştirmesi.
  • Proksimal humus kırığı — omuz çıkığına büyük tüberkül, daha seyrek olarak humerus boynu veya küçük tüberkül kırığı eşlik edebilir.

Normalde

Normal aynı düzey aksiyel BT'de humerus başı glenoid fossanın merkezinde, eklem aralığı düzenli ve iki eklem yüzü karşılıklıdır. Anteroinferior glenoid konturunun kesintisizliğini ve posterolateral humerus başı korteksinin düzgün yuvarlaklığını karşı tarafla kıyaslayın; çıkıkta hizalanma kayar, eşlik eden impaksiyon/fragman konturu bozar.
