Özet

Amfizematöz piyelit, gaz oluşturan enfeksiyonda gazın renal pelvis ve kalikslerle sınırlı kaldığı toplayıcı sistem enfeksiyonudur. Kontrastsız BT gazı duyarlı biçimde gösterir ve gazın yalnız lümende mi yoksa böbrek parankimine de mi uzandığını belirler. Parankim gazı veya nekroz saptanırsa bu sınırlı tablo amfizematöz piyelonefrit spektrumuna girer. İyatrojenik hava ya da fistülden gelen gazın enfeksiyon sanılması ve parankim uzanımının gözden kaçması acil durumun yanlış sınıflanmasına neden olabilir.

Renal pelvis ve kalikslerde gaz odakları görülür; gazın yalnızca toplayıcı sistemde biriktiği ve parankime yayılmadığı kanıtlanmalıdır.

BT bulguları

  • Toplayıcı sistem içi gaz — pelviste veya kalikslerde lümene uyan hava kabarcıkları kontrastsız kesitlerde belirgindir.
  • Parankim gazı yok — korteks, medulla ve perinefrik dokuda hava saptanmaması izole piyeliti destekler.
  • Genişlemiş pelvis ve kalikslerde gaz odakları görülür; gazın yalnızca toplayıcı sistemde biriktiği gösterilmelidir.
  • Renal pelvis duvarı — BT, renal parankimi değerlendirmeye ve gazı sıvı ya da taşlardan ayırmaya yardımcı olur.
  • Üriner obstrüksiyon — üreter taşı, hidronefroz veya genişlemiş kaliksler enfeksiyonun drenajını bozabilir.
  • Komşu enfeksiyon bulguları — mesane duvarı değişikliği ya da üreter çevresinde inflamasyon eşlik edebilir.
  • Gazın işlem/fistül ilişkisi — yakın zamanda kateterizasyon veya üriner-enterik ileti öyküsü varsa lümendeki hava enfeksiyondan bağımsız olabilir.

Normalde

Renal pelvis ve kalikslerde gaz görülmesi amfizematöz piyeliti düşündürür; gazın pyelokaliksiyel sistemde mi yoksa renal parankimde mi bulunduğu BT ile değerlendirilir. Gaz odağını kaliks veya pelvis lümenine göre haritalayın ve aynı böbreğin korteks-medullasını, renal sinüsü ve perinefrik yağı kesit kesit inceleyerek gazın parankim dışına taşıp taşmadığını belirleyin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
