Özet

Travmatik torakolomber subluksasyon, komşu vertebralar arasında kısmi translasyon veya rotasyonel hizalanma kaybıdır ve çoklu kemik ya da diskoligamentöz yapı hasarını yansıtabilir. BT, posterior korpus çizgisi, faset eklemleri ve posterior elemanları birlikte göstererek kaymanın düzlemini ve eşlik eden kırıkları ortaya koyar. BT'de faset uyumsuzluğu, spinöz çıkıntı kırığı ve interspinöz aralık genişlemesi PLC yaralanmasının dolaylı işaretleri olabilir; klinik şüphe sürüyorsa PLC bütünlüğünü değerlendirmede MR yardımcı olabilir. Travmatik translasyonun yanı sıra PLC bütünlüğü ve nörolojik durum da morfoloji sınıflamalarında belirleyicidir; bu bileşenlerden birinin ihmal edilmesi yaralanmanın stabilitesini ve tedavi gereksinimini yanlış değerlendirmeye yol açar.

Ölçütler ve sınıflamalar

AO Spine torakolomber morfoloji
AO Spine C tipi, spinal translasyonla (yer değiştirmeyle) tanımlanır. Posterior gergi bandı (tension-band) yaralanmaları AO Spine B tipi olarak sınıflanır; eşlik eden vertebra korpus lezyonları ayrıca A alt tipleriyle belirtilir.
TLICS yaralanma morfolojisi
TLICS sistemi, yaralanma morfolojisi, posterior ligamentöz kompleks (PLC) durumu ve nörolojik defisit olmak üzere üç bağımsız değişkene puan vererek toplam skoru hesaplar; travmatik kayma morfoloji kategorilerinden birini oluşturur. BT morfolojiyi gösterir, klinik muayene nörolojik bileşeni tamamlar.

Normalde

Şüpheli segmentte faset eklemi uyumsuzluğu ile vertebral translasyon veya rotasyonu araştırın; bunlar BT'de posterior ligamentöz kompleks yaralanmasının dolaylı bulguları olabilir.

Korpus burst kırıkları vertebral gövde parçalanmasını, spinal epidural hematomlar ise spinal kanalda yer kaplayan yumuşak doku lezyonunu gerektirir; bunlar tek başına çok seviyeli rotasyon bulgusuyla açıklanamaz.
